Child Care Foundational Training was formerly called “Entry Level.”

If your course was completed before July 2000, contact the agency where you completed the course to request a copy of your training certificate. The Wisconsin Registry does not have Entry Level records prior to July 2000.
Do not use this form if your course was completed at a college/university or in high school. The Wisconsin Registry cannot create these documents.
We encourage you to access these certificates electronically, but if you choose to fill out this form, processing and mailing of your printed certificate will take 2-4 weeks after the form is received.
