Registry Certificate of Achievement Print Request

INSTRUCTIONS:
  1. Complete all sections form below and click the SUBMIT button. You will receive a confirmation email from support@wiregistry.org.
  2. Please list an email address that is currently being used and checked frequently.
  3. List your name exactly as you would like to have it appear on your Registry Certificate of Achievement and on your Registry account.
  4. The Registry Certificate of Achievement will be mailed to the address provided on this form. 

After the form is received, processing and mailing of your printed certificate will take 2-4 weeks. A Registry Certificate of Achievement can only be printed while a Registry membership is current.

Beginning on November 6, 2025, all Wisconsin Registry users will see a new and improved Personal Profile when logging in to the Wisconsin Registry system. This enhanced layout ensures that your information is organized in a more user-friendly, mobile-first format. All functionality will remain the same.
